Structure

Imikhimod, acid isostearin, cetyl alcohol, paraffin white soft, stearyl alcohol, polysorbate, benzyl alcohol, sorbitan stearate, gum xanthane, propilparagidroksibenzoat, glitserol, the water purified.

Pharmacodynamics

The main active ingredient of drug belongs to the class of imidazokumolinamin and is the inductor of immune system. At the same time, Imikhimod does not make direct impact on a virus, and acts as the antineoplastic agent by activation of immune system through TLR7 (toll-like receptor 7).

It is also effective against viral infections. Imikhimod activates NK and B-cells, and also Langergans's cells. The main result of effect of drug is increase in production of cytokines: interleykina-6 (IL-6), alpha interferon (IFN-α) and TNF-α (factor of a necrosis of tumors). Cytokines, respectively, stimulate an immune response and increase antiviral activity.

Imikhimod also has anti-proliferative (cytostatic) effect. Stimulating development of a receptor of OGFR (an opioid growth factor), drug slows down growth of cells of malignant tumors.

Pharmacokinetics

At outside use through skin less than 0,9% of the put drug dose get. The insignificant quantity of an imikhimod which gets to the general blood stream to be output during a short span through urine and kcal, and its concentration in blood does not reach quantitatively defined level (> 5 ng/ml). Cmax of an imikhimod in blood comes within 10 hours and makes when drawing on face skin 1,6 ng/ml.

Indications to use

• Treatment of the outside sharp-pointed condylomas which are localized on perianal area and on skin of external genitals;
• In therapy of a bazalioma and actinic keratosis at patients with normal function of immune system.

Contraindications

Age till 18 flyings, high sensitivity to drug, pregnancy and the period of a lactation.

Side effects

Dizziness, headache, nausea, vomiting, pain and itch of skin in drug sites of application, erythematic rash, a folliculitis, diarrhea, increased fatigue, a mialgiya, grippopodobny symptoms, an arthralgia, fever, sleeplessness or drowsiness, a depression, irritability, pharyngitis, rhinitis, a back pain, perspiration, an abdominal pain, anorexia, local hyperpegmentation, a vaginitis, a herpes simplex, decrease in hemoglobin, a leukopenia.

Aldara, application instruction (Way and dosage)

Aldar's (aldara) cream is applied outwardly, it is applied on the cleared condyloma surface with a thin layer and completely rubbed in skin. It is necessary to avoid hit of cream on a skin surface outside an affected area, and also to cause its excess quantity.

After putting drug it is impossible to apply a bandage, to leave on skin about 10 hours, during this period it is impossible to accept hydrotherapeutic procedures. After the specified term cream should be washed away water with detergent. The mode of putting drug — 3 times a week once a day before going to bed. To continue treatment before disappearance of visible condylomas, however no more than 4 months. One package of Aldar is expected approximately the area of skin of 20 cm2.

Overdose

Use of drug in the doses exceeding therapeutic increases risk of development of local reactions in severe forms; At accidental hit of an imikhimod inside in a dosage it is higher than 200 mg (about 16 bags), vomiting, nausea, a mialgiya, a headache, fever can be noted. In hard cases — arterial hypotension.

Interaction

Extremely low absorption of drug through integuments limits interaction with HP of systemic action. To use drug with care to the patients receiving drugs immunosuppressors.
