Structure

Pharmacodynamics

Treats group of the selection TsOG-2 inhibitors (cyclooxygenase-2). Drug has analgeziruyushchy, antiinflammatory and febrifugal effect. The mechanism of action is based on reduction in the center of an inflammation of synthesis of prostaglandins, and also in a hypothalamus. In kidneys and a mucous membrane of a stomach drug practically does not influence synthesis of prostaglandins, than its good tolerance speaks. Drug oppresses process of peroxide oxidation of fats and education of free radicals, reduces development of a factor of activation of thrombocytes, release of a histamine, factor of an inflammation, proteinases and other mediators of an inflammation.

Pharmacokinetics

Drug is well soaked up from a gastrointestinal tract, meal reduces the speed of absorption. In an organism does not kumulirut. Therapeutic concentration of drug in blood is defined in 2 hours and is maintained for 8 hours. An elimination half-life — 3 hours. Biotransformation occurs in a liver to level pharmacological active (hydroxynimesulide) and inactive metabolites. It is removed through kidneys and intestines.

Indications to use

Stopping of a pain syndrome of various origin, fever of various genesis, inflammatory diseases of joints and muscles (arthritises, miozita, osteoarthrites, tendovaginitis).

Contraindications

High sensitivity to drug, aggravations of cankers of a gastrointestinal tract, a renal failure, bleeding in a gastrointestinal tract, a liver failure, the III trimester of pregnancy, a lactation, medicinal dependence, alcoholism, fervescence, age till 12 years, blood coagulation disturbances.

Side effects

Dizziness, headache, drowsiness, pains in a stomach, heartburn, an ulceration mucous a gastrointestinal tract, nausea, diarrhea, hypostases, an oliguria, a hamaturia, an agranulocytosis, thrombocytopenia, local and general allergic reactions.

Aponil, application instruction (Way and dosage)

For adults the single dose varies from 50 to 200 mg, maximum — 400 mg a day. About 1 tablet after food 2 times a day is accepted, it is washed down with water. A dosage for children — 1,5 mg/kg (the maximum dose — 5 mg/kg a day).

Overdose

In rare instances nausea and vomiting.

Interaction

At a concomitant use of drug with indirect anticoagulants the risk of strengthening of their action increases. At the combined reception with lithium salts their concentration in a blood plasma increases. Nimesulide is competitively forced out from communication with proteins by gipoglikemiziruyushchy means, acetylsalicylic acid, streptocides, cyclosporine, foxglove drugs, a methotrexate.
