Structure

Foroz's drug includes 70 mg of alendronovy acid. Additional components: silicon dioxide, MKTs, croscarmellose sodium, Lustre Clear LC103, magnesium stearate.

Release form

Biconvex white tablets of a round form with a text of "ALN 70" on one party.

• 2 tablets in the blister — 1, 3, 2, 4 or 6 blisters in a pack from a cardboard;
• 4 tablets in the blister — 1, 3, 2, 4 or 6 blisters in a pack from a cardboard.

Brakes a bone resorption.

Pharmacodynamics and pharmacokinetics

Pharmacodynamics

The specific non-hormonal inhibitor of a bone osteoklastichesky resorption oppressing activity of osteoclasts. Drug stimulates bone formation, recovers the correct balance between destruction and recovery of a bone tissue, increases density of bones (normalizes a phosphorus-calcium exchange), facilitates growth of a bone tissue with physiologic histologic structure.

Pharmacokinetics

Bioavailability at oral administration (on an empty stomach) fluctuates within 0,64-0,59%. At the use of an alendronat together with food and within 120 minutes after food bioavailability is insignificant. At a concomitant use with orange juice or coffee it also decreases by 60%. Content of drug in blood is not enough. Linkng with proteins of plasma makes 78%.

The data confirming biotransformation of an alendronat in a human body no. Time of semi-removal comes by 72 o'clock. With urine about a half of the entered dose and small quantity – with a stake is removed.

Indications to use

• therapy of osteoporosis from prevention of changes at men;
• therapy of osteoporosis in a postmenopause at women, and also for prevention of compression fractures of vertebras and fractures of a neck of a femur at this group of patients;
• therapy of the osteoporosis caused by long reception of glucocorticosteroids.

Contraindications

• allergy to drug components;
• strictures or disturbance of passability of a gullet;
• inability of the patient to stand or sit within half an hour;
• the expressed frustration of an exchange of minerals;
• hypocalcemia;
• heavy insufficiency of work of kidneys;
• pregnancy and lactation;
• the age is younger than 18 years.

Foroz's tablets should be applied with care to persons with diseases of the digestive system (gastritis, a dysphagy, after an operative measure on bodies of upper parts of the alimentary system, an aggravation of a peptic ulcer, a duodenitis, gastrointestinal bleeding, a hypovitaminosis of D).

Side effects

• Reactions from digestion: melena, dyspepsia, esophagitis, abdominal pains, heartburn, dysphagy, nausea, vomiting, gastritis, bleeding and ulceration of mucous upper parts of the alimentary system, gullet stricture, perforation of a gullet.
• Reactions from nervous activity: headaches, food faddism, dizziness, irritability.
• Reactions from sight: sclerite, uveitis, inflammation of connecting fabric of episkleralny area.
• Reactions from skin: Quincke's disease, dermahemia, alopecia, Stephens-Johnson's syndrome, small tortoiseshell, photosensitivity, epidermal toxic necrolysis.
• Reactions from skeletal and muscular system: changes of an upper third of a femur, muscle pain, swelling of joints, ostealgia, osteonecrosis of a jaw, joint pain.
• Laboratory indicators: symptomless temporary hypophosphatemia and hypocalcemia.

Foroza, application instruction (Way and dosage)

Drug is accepted orally.

For the purpose of ensuring the best absorption of drug it needs to be used in the morning on an empty stomach, washing down with a glass of water and not earlier, than for half an hour before the first reception of food, drinks or other medicines. Other drinks (including mineral water) are capable to lower absorption of active ingredient.

For lowering of irritation mucous a gullet, Foroz's pill needs to be taken:

• only after final awakening from a dream and a rising from a bed;
• swallowing entirely (without chewing, not rassasyvy and without dissolving them in an oral cavity because of risk of formation of ulcers;
• it is forbidden to adopt the lying provision before the first reception of food which follows not earlier, than in half an hour;
• it is forbidden to take medicine before going to bed or after a dream before rise from a bed.

Standard dosage of drug of Foroz – 70 mg once a week.

The dosage is not subjected by corrections at elderly persons and patients with diseases of a liver and kidneys. Persons with the expressed disorder of work of kidneys are not recommended to appoint drug.

Overdose

Overdose signs: dyspepsia, hypocalcemia, gastritis, abdominal pains, heartburn, dysphagy, esophagitis, hypophosphatemia.

Overdose treatment: symptomatic; reception of milk and antacids for binding of an alendronat. Because of high risk of an ulceration of a gullet it is forbidden to cause vomiting, the patient has to be in vertical position.

Interaction

Joint administration of drugs of calcium or antacids suppresses absorption of an alendronat. Therefore it is recommended to accept other medicines at least half an hour later after Foroza's reception.

Anti-inflammatory nonsteroid drugs (including Aspirin) can strengthen undesirable effects of drug from digestion.

Use of an alendronat with a large number of the most used drugs was not followed by emergence of interaction significant clinically.

To store in the dry dark place at a temperature up to 25 °C. To protect from children.

Period of validity

3 years.

Special instructions

Tablets can be washed down only with boiled water because other drinks (for example, tea, mineral water, coffee, juice) slow down Foroza's absorption.

Use of an alendronat before going to bed or in a prone position increases probability of development of an esophagitis.

Before an initiation of treatment drug it is necessary to make correction of a hypocalcemia and other frustration of a metabolism (for example, a lack of vitamin D). Because of increase in density of bones against treatment alendronaty insignificant asymptomatic lowering of content of calcium and phosphates in blood is not excluded. Therefore it is necessary to provide receipt of a sufficient dose of calcium and vitamin D, especially at the persons receiving glucocorticosteroids.

Patients need to be warned that at the admission of reception of the next tablet it is necessary to accept it in the morning of the next day (it is forbidden to take 2 pill in a day). In the subsequent it is recommended to continue to accept drug in the day of the week chosen in an initiation of treatment.

Seldom at use of other bisfosfonat the jaw osteonecrosis came to light. The risk factors provoking emergence of an osteonecrosis of a jaw: a malignant tumor, the accompanying treatment (radiation therapy, chemotherapy, corticosteroids), diseases of gums, an infection, a coagulopathy, anemia. The majority of similar cases is registered at intravenous administration of bisfosfonat.

Stomatologic operative measure against treatment of a bisfosfonatama is capable to aggravate a current of an osteonecrosis of a jaw.
