Active component Glibenclamidum – 5 mg is a part of drug.

Glibenclamidum in the form of the tablets packed on 120 pieces into a bottle which is placed in a pack is issued.

Pharmacological action

Drug possesses antitrombotichesky, hypolipidemic and hypoglycemic action.

Pharmacodynamics and pharmacokinetics

Glibenclamidum is the peroral hypoglycemic drug relating to sulfonilmochevina derivatives. The mechanism of action of Glibenclamidum assumes insulin secretion stimulation by β-cells in a pancreas, increasing insulin release. Mainly efficiency is shown in the second stage of production of insulin. At the same time sensitivity of peripheral fabrics to effect of insulin, and also its communication with target cells increases. Besides, hypolipidemic effect and lowering of thrombogenic properties of blood is characteristic of Glibenclamidum.

In an organism fast and full-fledged absorbability of substance from a gastrointestinal tract is noted. Communication with proteins of plasma corresponds to nearly 95%. The metabolism of drug is carried out in a liver therefore inactive metabolites are formed. Removal happens mainly in composition of urine and part – bile, in the form of metabolites.

Indications to use

Glibenclamidum is appointed to patients at a diabetes mellitus 2 types when it is impossible to compensate a hyperglycemia by a diet, decrease in body weight and physical activity.

Contraindications to use

Drug is not recommended for reception at:

  • hypersensitivity to Glibenclamidum or streptocides and thiazide diuretics;
  • diabetic prekomatozny or coma;
  • ketoacidosis;
  • extensive burns;
  • injuries and surgical interventions;
  • intestinal impassability and paresis of a stomach;
  • various forms of disturbance of absorption of food;
  • development of a hypoglycemia;
  • lactations, pregnancies;
  • diabetes mellitus of 1 type and so on.

Side effects

At treatment by Glibenclamidum development of the undesirable symptoms affecting work endocrine digestive, nervous, peripheral and systems of a hemopoiesis is possible. Therefore it can be shown: hypoglycemia of varying severity, nausea, diarrhea, disturbance of functions of a liver, cholestasia, headache, weakness, fatigue and dizziness.

Also manifestation of allergic and dermatological reactions in a look is not excluded: skin rash, itch, photosensitization and other symptoms.

Application instruction of Glibenclamidum (Way and dosage)

The application instruction of Glibenclamidum reports that the dose of drug is established individually and depends on age, disease severity and level of a glycemia. A pill is taken inside, on an empty stomach or later 2 h after the use of food.

The average daily dose is established within 2,5-15 mg, with a frequency of reception of 1-3 times a day.

Daily doses higher than 15 mg use seldom, at the same time considerable strengthening of hypoglycemic effect is noted. Establish to elderly patients in an initiation of treatment a daily dose of 1 mg. All transitions from one drug to another, manipulations with a dose and so on, it is necessary to carry out under control of the specialist.

Overdose

In case of overdose development of a hypoglycemia which is capable to be followed by feeling of hunger, weakness, concern, a headache, dizziness, perspiration, heartbeat, a muscular shiver, wet brain, an alalia and sight and so on is possible.

Treatment assumes urgent reception of sugar, fruit juice, sweet hot tea, corn syrup, honey – in mild cases.

Hard cases demand administration of solution of glucose of 50%, continuous infusion in a vein of solution of the Dextrose of 5-10%, introduction of the Glucagon intramusculary, Diazoxide inside. Besides, control of level of a glycemia, definition of pH, creatinine, an urea nitrogen, electrolytes is necessary.

Interaction

The combination to antifungal drugs of systemic action, ftorkhinolonama, tetracyclines, chloramphenicol, H2 blockers, beta adrenoblockers, APF and MAO inhibitors, Clofibratum, bezafibraty, probenetsidy, Paracetamol, Etioniamidum, anabolic steroids, pentoksifilliny, Allopyrinolum, cyclophosphamide, Reserpinum, streptocide and Insulin is capable to exponentiate a hypoglycemia.

Simultaneous use with barbiturates, fenotiazina, diazoxide, glucocorticoid and thyroid hormones, estrogen, gestagena, glucagons, adrenomimetichesky drugs, the lithium salts derivative of niacin and saluretics can weaken hypoglycemic effect.

The means capable to acidify urine, for example: Calcii chloridum, ammonium chloride, high doses of ascorbic acid can strengthen effect of drug. Combinations with Rifampicin accelerate an inactivation and reduce its efficiency.

Special instructions

It is recommended to carry out with care treatment of the patients suffering from disturbances of a liver and kidneys at feverish conditions, pathological functioning of adrenal glands or a thyroid gland, an alcoholism.

Careful control of level of glucose as a part of blood and excretion of glucose is necessary for course of full therapeutic process.

If the hypoglycemia at patients in consciousness develops, then sugar or glucose are appointed inside. In cases of a loss of consciousness glucose is entered intravenously, and a glucagon – intramusculary, subcutaneously or intravenously.

When the consciousness is recovered, the patient is given the food saturated with carbohydrates at once to avoid a repeated hypoglycemia.
