Structure

Nebivolola hydrochloride, beta cyclodextrin, croscarmellose sodium, lactose, magnesium stearate, sodium sulfate dioctyl, silicon dioxide, microcrystallic cellulose, talc, povidone.

Release form

Tablets 2,5; 5 and 10 mg in the blister in cardboard packaging No. 15, 50, 105.

Pharmacological action

Antiarrhytmic, anti-anginal, hypotensive.

Pharmacodynamics and pharmacokinetics

Pharmacodynamics

Drug is cardioselective beta1-adrenoblocker, having vazodilatiruyushchy effect. Blocks beta1-adrenoceptors, transforms process of development of the endothelial running-down factor. Decrease in the ABP is caused by reduction of OPSS, cordial emission, OTsK, braking of process of formation of a renin, lowering of sensitivity of baroreceptors.

As a rule, the hypotensive effect is shown in 1–2 weeks, and its stabilization — for a month. Nebilong lowers ChSS at loading and at rest, reduces oxygen requirement of a cardiac muscle, improves diastolic filling, reduces the weight and an index of mass of a myocardium, exerts beneficial effect on a day-night rhythm of the ABP. Does not influence a lipidic exchange.

Pharmacokinetics

Drug is quickly absorbed from a gastrointestinal tract. High communication with blood proteins (98%). Gets through GEB. It is metabolized in a liver. Are allocated in the form of glucuronides with urine and a stake.

Indications to use

Arterial hypertension, prevention of attacks of an angina of exertion, ischemic heart disease, in the combined treatment of HSN.

Contraindications

High sensitivity to drug, bronchial asthma, diseases of a liver, HSN in a decompensation stage, a pheochromocytoma not treated, angiospastic stenocardia, arterial hypotension, depressions, cardiogenic shock, bradycardia, a myasthenia, age till 18 flyings.

Side effects

Feeling of fatigue, dizziness, headache, paresthesias, depression, drowsiness, decrease in concentration of attention, sleeplessness, hallucinations, nausea, xerostomia, orthostatic hypotension, diarrhea/lock, hypostases, asthma, bradycardia, local allergic reactions, cardialgia, arrhythmias, rhinitis, bronchospasm, hyperhidrosis, photodermatosis.

Nebilong, application instruction (Way and dosage)

To accept inside in the morning adult in a dose of 2,5-5 mg a day. According to indications it is possible to increase a dose to 10 mg a day. To patients of advanced age the initial dose makes 2,5 mg a day which according to indications it is possible to increase to 5 mg.

Overdose

It is shown by arterial hypotension, bradycardia, acute heart failure, a bronchospasm.

Interaction

At a concomitant use of drug with cardiac glycosides of strengthening of influence on delay of atrioventricular conductivity it is not revealed. The concomitant use of Nebilonga with HP for the general anesthesia increases probability of development of arterial hypotension and suppresses reflex tachycardia.

At a concomitant use of drug with Cimetidinum concentration of Nebilonga in blood increases. The medicamentous means suppressing the return serotonin reuptake slow down a metabolism of Nebilonga. Sympathomimetics reduce activity of Cimetidinum. Barbiturates, tricyclic antidepressants can strengthen hypotensive action.

Pregnancy and lactation

Administration of drug during pregnancy is made only according to strict indications because of high risk of development in newborns of arterial hypotension, bradycardia, paralysis of breath, a hypoglycemia. Reception of Nebilonga should be stopped in 2-3 days prior to the expected term of childbirth. If it is impossible or childbirth came strict control of a condition of the newborn child within the first three days of life is prematurely necessary.
