Structure

Solution: an ondansetrona of a hydrochloride a dihydrate, sodium citrate, monohydrate of citric acid, sodium chloride, water for and.

Tablets Osetron: ondansetrona of a hydrochloride dihydrate, corn starch, magnesium stearate, lactose, silicon dioxide colloid, propylene glycol, titanium dioxide, gipromelloza, talc.

Pharmacodynamics

Drug with the expressed antiemetic action. Is an antagonist of receptors, competitive with sharp selectivity, 5HT3 (the serotoninovy receptors which are located in the central and peripheral nervous system). At reception of chemotherapeutic remedies and sessions of radiation therapy serotonin is released from enterokhromafinny cells mucous by a gastrointestinal tract that causes emergence of an emetic reflex and feeling of nausea. Drug selectively blocks membranes of receptors 5HT3 of peripheral nerves which actively participate in this reflex. Thus, drug stops nausea and vomiting, appearing in process himio-and radiotheraphy, at the same time, any expressed collateral extrapyramidal manifestations are absent.

Pharmacokinetics

Drug is quickly soaked up in a gastrointestinal tract, the maximum level of concentration in blood after introduction in oil is reached in 1,5 hours. Linkng with blood proteins about 75%. Biotransformation occurs in a liver by hydrolysis. Are removed drug mainly in the form of metabolites through kidneys and intestines.

Indications to use

• As the medicine stopping the nausea and vomiting caused by carrying out cytotoxic radio / himio therapies;
• Prevention and stopping of nausea and vomiting after operations.

Contraindications

High sensitivity to drug, the period of pregnancy and a lactation, a fenilketonuriya, age till 2 years, in the presence of the extended inborn interval of QT.

Side effects

Local and general allergic reactions, dryness in a mouth, diarrhea or locks, bradycardia, thorax pains, decrease in the ABP, arrhythmia, dizziness, motive frustration, a headache, spasms, feeling of heat, a hypopotassemia, visual acuity disturbance.

Osetron, application instruction (Way and dosage)

The dosing mode at cytostatic therapy individual is also defined by expressiveness of emetic reaction. The daily dose of Osetron varies from 8 to 32 mg.

Osetron's solution for injections is entered intramusculary or intravenously. For preparation of solution for infusion Ringer's solution is used, sodium of chloride or glucose, solution is prepared before introduction.

• At moderately emetogenny radio/chemotherapy in 2 hours prior to a session — 8 mg of drug and intake in 12 hours in oil or in/in 8 mg slowly, before therapy;
• At highly emetogenny radio/chemotherapy — intake in 2 hours prior to a session of 24 mg of drug together with dexamethasone in a dose of 12 mg or in/in slowly 8 mg before procedure, and then in 2–4 hours 2 in/in an injection on 8 mg or continuous infusion of drug within a day, before procedure in a dose of 24 mg with a speed of 1 mg/hour;
• For the purpose of prevention of late vomiting, it is necessary to continue administration of drug within 5 days in a dose of 8 mg twice a day.

For children after 2 flyings Osetron's dosage — 5 mg/sq.m in / century. The drug is administered before radio/chemotherapy, it is accepted inside in 12 hours of 4 mg. Administration of drug inside should be continued for 5 days in a dose 4 mg twice a day.

Prevention of nausea and vomiting after operations. The single dose to adult patients — 4 mg to enter in oil or in/in slowly struyno in an initial grade of anesthesia or to take inside for 1 h prior to the beginning of the general anesthesia in a dose 16 mg. For stopping of the arisen vomiting in oil in/in introduction to 4 mg of drug. To children for the purpose of prevention of nausea after operation the drug is administered parenterally in a dosage of 0,1 mg/kg to or after an anesthesia.

Overdose

It is shown by manifestation of side effects.

Interaction

Care when sharing drug with barbiturates is necessary, glyutetimidy, a papaverine, griseofulvin, phenylbutazone, rifampicin, karizoprodoly; makrolidny antibiotics, oral estrogensoderzhashchy contraceptives, eritromitsinom, omeprazoly, metronidazole, ketokonazoly, Cimetidinum, quinine, propranolol, quinidine and other HP.
