Correction of a form and sizes of auricles (otoplasty)
December 13, 2011
Otoplasty (from the Greek othos, i.e. an ear) are plastic surgeries on auricles. Most often the otoplasty is understood as change of the bulged auricles. In a case with a normal ear the corner between the head and an auricle makes about 30 degrees. And the bulged ear can make with the head a corner to 90 degrees. Usually width of an ear makes about 60% of its height, with a height of 60 mm.
Main objective of an otoplasty is change of position of an auricle concerning the head or correction of a form of a sink, for achievement of its proportionality and improvement of outward. Many children suffer from a lopoukhost which leads to the fact that peers begin to deride this their shortcoming. And though it is considered that the bulged ears usually happen at the people who are intellectually developed and creative, most often they break appearance of the person that can lead to development of a stress.
Indications and contraindications to an otoplasty
Indications to an otoplasty:
- macrotia and microtia;
- irregular shape or disproportion of auricles (the flattened auricles, sharp-pointed shape of ears);
- the grown auricles, thick ears;
- recovery of an auricle or the torn-off lobe.
It is the best of all to perform operation at children's age, till 6 flyings when growth of an ear is complete, before school life in order to avoid possible sneers of schoolmates.
However to carrying out an otoplasty there are also contraindications. The main contraindications to an otoplasty a dekompensirovanny diabetes mellitus, disturbances of coagulability of blood, a disease of internals, including chronic infectious diseases, and also some mental and neurotic disorders.
Indications to an otoplasty at a lopoukhost the most frequent. Treatment by surgical methods was carried out in the 1840th years.
The main objective of an otoplasty is correction of protrusion of upper part of an ear. The main methods of carrying out plastics of auricles are reduction of its size, removal of skin behind an ear (Mustarde's method), and also weakening of a cartilage of an antihelix and giving of a form to it pleated.
Methods of performance of an otoplasty
The best and safest method of an otoplasty is the method of Mustarde at which skin behind an ear is removed and stitches on a cartilage for giving of a necessary form to it are put. Sometimes at surgical treatment of the bulged ears doctors use a method of FurnASA at which weakening of a cartilage and imposing of special seams which delay a sink back is made. At such type of an otoplasty the auricle cartilage often gets to outside acoustical pass that demands further carrying out a resection of a cartilage.
Usually the otoplasty is carried out under local anesthesia or the general anesthesia and to last 1-2 hours. When carrying out operation the section is made by back part of an ear, the cartilage becomes thinner drawing notches then it is fixed in new situation. Then seams and a compression wadded gauze bandage are applied on skin. Because of an arrangement of hems in a natural fold of back part of an auricle, they are not noticeable.
In case of carrying out operation on recovery of an ear cartilage, the plastics with use of the cartilaginous tissues taken from edges or local fabrics is carried out. Stages of this type of an otoplasty consist of production and the room of a cartilage in a skin pocket then within 4 months recovery of a lobe of an ear and a raising of an auricle is carried out, and right at the end create deepening of an auricle. Depending on a condition of an auricle stages of correction can integrate.
Operations on reduction of the sizes of an auricle are performed by method of geometrically measured removal of an ear cartilage.
Usually for carrying out such type of operation if there are no contraindications to an otoplasty, it is necessary 1 day is in hospital under supervision of doctors, and seams are removed approximately in a week. After completion of operation 2 more weeks it is necessary to carry a soft bandage for fixing of a cartilage (impregnated with ointment antibiotic), and the elastic bandage on an ear is carried within 1 month. Hypostasis and bruises usually fall down in 3-5 days. During this period it is impossible to be engaged in heavy physical exercises and activity which can lead to injuries of ears.
Are distinguished from complications of an otoplasty inadequate correction which demands carrying out new operation, and also a hypertrophy of cicatricial fabrics.
