Structure

The tablet contains 2 active agents in a film cover: paracetamol (500 mg) and caffeine (65 mg). Similar structure at a soluble tablet.

Additional components: stearic acid, polividon, prezhelatinizirovanny starch, talc, triacetin, gidroksipropilmetiltsellyuloza, water, kroskarmelloz sodium, potassium sorbate.

Release form

Tablets in a special white film cover with flat edges of a kapsulovidny form. On one of the parties the text of "PANADOL EXTRA" is engraved. The blister contains 12 tablets. In a cardboard pack there is an instruction and 1 blister.

Soluble tablets are packed into strips on 2 pieces. In a pack from a cardboard there are 6 strips and the instruction.

Pharmacological action

The combined medicine which pharmacological influence is caused by effects of two active components: paracetamol and caffeine.

Caffeine. Has psychogogic effect which is caused by impact on the psychomotor centers in a brain. Caffeine eliminates feeling of fatigue and drowsiness, strengthens action of analgetics, increases intellectual working capacity and physical endurance.

Paracetamol. Has analgeziruyushchy and antipyretic effects. The anesthetizing action is reached due to oppression of processes of synthesizing of prostaglandins, at the expense of Cyclooxygenase enzyme inhibition. The analgetic effect is caused by ability of Paracetamol to influence carrying out painful impulses in a nervous system and to influence the center of thermal control in a hypothalamus.

Pharmacodynamics and pharmacokinetics

After reception of per os Paracetamol is well absorbed from a gleam of a digestive tract. In 0.5-2 hours after oral administration the peak of plasma concentration is registered. Active ingredient is metabolized in hepatic system. The indicator of an elimination half-life is equal to 1-4 h. A preferential way of removal – through renal system with urine in the form of metabolites.

Indications to use Panadolum Extra

The medicine is applied to stopping of a pain syndrome of various etiology (average and moderate expressiveness):

• migraine attacks;
• neuralgia;
• muscular pain;
• headache;
• algodismenoreya;
• arthralgia;
• pains of rheumatic genesis.

Panadolum Extra can be appointed as antipyretic at a SARS, flu, cold and infectious diseases.

Contraindications

• glaucoma;
• the expressed atherosclerotic manifestations;
• diseases of system of blood;
• the expressed damages of kidneys / hepatic system;
• individual hypersensitivity;
• constitutional hyperbilirubinemia;
• pregnancy;
• genetic deficit dehydrogenase glyukozo-6-phosphate;

Side effects

Reception of a medicine in the dosages recommended by the producer seldom causes negative symptomatology:

• Quincke's edema;
• allergic answers;
• skin itch;
• thrombocytopenia;
• tachycardia;
• sleep disorders;
• agranulocytosis;
• methemoglobinemia;
• leukopenia;
• rashes on integuments.

Prolonged treatment by high dosages increases risk of nephrotoxic and hepatotoxic defeat. It is obligatory to control blood indicators. At registration of other negative reactions during treatment the termination of reception of tablets and the address to the treating doctor is recommended.

Panadolum Extra, application instruction (Way and dosage)

Tablets are intended only for reception of per os. According to the standard scheme the medicine is appointed by 3-4 times a day on 1-2 tablets. Application instruction Panadolum Extra: it is recommended to maintain the minimum time interval between receptions of tablets, to equal 4 hours. It is one-time possible to take no more than 2 pill. In days no more than 8 tablets are allowed to reception.

As an analgetic Panadolum Extra can be applied no more than 5 days, and as antipyretic – not longer than 3 days. According to the decision of the doctor duration of therapy and a daily dose of a medicine can be increased.

In the absence of the expected positive symptomatology consultation of the doctor is required. Independent exceeding of the dosages specified by the producer is not recommended. At reception of the raised doses of a medicine consultation of the treating doctor, even in the absence of negative symptomatology is obligatory (development of the remote effects connected with hepatotoxic influence of one of active components – paracetamol is possible.)

Overdose

Reception of high doses strengthens risk of manifestation of gapatotoksichesky or nephrotoxic influence of a medicine. Disturbances from a nervous system in the form of the following manifestations are possible:

• tremor of extremities;
• hyperexcitability;
• disturbances of the mode of dream/wakefulness;
• dizzinesses.

Manifestations from system of a hemopoiesis:

• pancytopenia;
• thrombocytopenia;
• anemia;
• neutropenia;
• leukopenia;
• agranulocytosis.

Other reactions:

• gepatonekroz;
• pallor of integuments;
• convulsive syndrome;
• tachyarrhythmia;
• development of a metabolic acidosis;
• disturbance of a sugar metabolism.

Therapy of poisoning is directed to a gastric lavage, use of enterosorbiruyushchy medicines (Polisorb, Smekt, Absorbent carbon, etc.), stopping of negative symptomatology. To patients in a serious condition intravenously pour in N-Acetylcysteinum, and appoint per os Methionine at safety of an opportunity to independently take a pill and in the absence of vomiting. Diazepam is applied to stopping of a convulsive syndrome.

Interaction

Action of indirect anticoagulating means (coumarins, Warfarin) amplifies at long administration of drug Panadolum Extra that can provoke bleedings. The risk of hepatotoxic manifestations amplifies at reception of inductors microsomal oksileniye of hepatic system:

• Phenylbutazone;
• Rifampicin;
• Phenytoinum;
• barbiturates;
• tricyclic antidepressants;
• Ethanol.

The risk of defeat of hepatic system decreases at purpose of Cimetidinum and other inhibitors of a microsomal oxidation. Panadolum Extra accelerates Hloramfeniol's removal (time of excretion increases by 5 times) due to effect of paracetamol. Absorption of Ergotamine accelerates at the expense of caffeine. The risk of development of acute pancreatitis and defeat of hepatic system increases at a concomitant use of alkogolsoderzhashchy drinks. Speed of absorption of one of active components of the drug Panadolum Extra — paracetamol — decreases at reception of Colestyraminum. The converse effect is observed at Domperidon's use and Metoclopramidum. Medicine reduces efficiency of uricosuric medicines.

Panadolum Extra is on sale in pharmaceutical points. It is optional to show the prescription form.

Storage conditions

The medicine has to be stored in original packaging. The temperature condition of storage recommended by the producer for preservation of efficiency of drug, from 15 to 25 degrees.

Period of validity

4 years.

Special instructions

It is not recommended to use coffee and tea in large numbers during treatment since various disturbances of a heart rhythm, tachycardia, sleeplessness, superexcitation can be shown. To persons with an alcoholism medicine is appointed with care.
