Structure

Active agent of Sonapaks which international name (MNN) – thioridazine, is thioridazine a hydrochloride which one tablet contains 10 mg or 25 mg.

Act as excipients: lactoses monohydrate, talc, silicon dioxide colloid, gelatin, starch corn, stearic acid.

Acacia gum, sucrose, talc, dye are a part of a cover.

Release form

Tablets are covered with a cover, round, have light pink color, a break – white color. Packaging of 20 tablets, in a cardboard box contains 3 packagings.

Pharmacological action

Tablets of Sonapaks have antipsychotic, antidepressive and tranquilizing effect.

Pharmacodynamics and pharmacokinetics

Active ingredient makes the central and peripheral impact on a nervous system. Substance is capable to suppress certain functions of a trunk of a brain, and also to render antihistaminic and hinolitichesky effect. Does not suppress motor activity, has no antiemetic action, estrapyramidal disturbances provokes less than other neuroleptics.

In addition thioridazine renders weak antidepressive, anti-autistic and antipsychotic action.

From intestines it is soaked up completely. Reaches the maximum concentration after reception in 2-4 hours. Kidneys remove about 35% of drug, the rest – through intestines (as in pure form, and in the form of metabolites). Through a gematoplatsentarny barrier thioridazine gets into breast milk.

Indications to Sonapaks's use

Administration of drug is appointed at the following diseases:

Contraindications

It is not necessary to accept drug in the following cases:

  • to children till 4 flyings;
  • liver failure;

Side effects

Medicine of Sonapaks can cause the following side effects:

  • agitation, sleeplessness, thermal control disturbance, syncope, confusion of consciousness, excitement, parkinsonism, decrease in a convulsive threshold, late dyskinesia, dystonic frustration, emotional disturbances, ZNS, extrapyramidal frustration;
  • cholestatic hepatitis, anorexia, diarrhea, the increased appetite, vomiting, hypoptyalism, nausea, hypertrophy of tongue nipples, dyspepsia, intestinal impassability;
  • tachycardia (including the pirouette type), pressure decrease, change of an ECG (including the lengthening of an interval of QT depending on the dose size);
  • leukopenia, pancytopenia, thrombocytopenia, agranulocytosis, granulocytopenia, aplastic anemia, eosinophilia;
  • rash, allergic reactions on skin, exfoliative dermatitis, an erythema, a bronkhospastichesky syndrome, a nose congestion, a Quincke's disease;
  • photosensitization, skin melanosis (develops at reception of high doses a long time);
  • increase in weight, false positive take of the test for pregnancy;
  • dysmenorrhea, inconscience with overflow, disturbance of an ejaculation, giperprolaktinemiya, dysuria, decrease libido, priapism, gynecomastia;
  • photophobia, vision disorder.

Application instruction of Sonapaks (Way and dosage)

Pill is taken inside, the dosing mode for each patient is set individually.

The adult at psychoses usually appoint in out-patient conditions 150-400 mg a day divided into 3-4 receptions in a hospital – 250-800 mg. It is necessary to begin treatment with low doses (in days of 25-70 mg), gradually the size of a dose increases to achievement of optimum effect (the antipsychotic effect is observed after an initiation of treatment in 10-14 days). Usually the course of treatment lasts several weeks. As a maintenance dose appoint 70-200 mg before going to bed once. To elderly people appoint lower doses – 100-300 mg. It is necessary to cancel drug gradually.

The therapeutic dose at slight emotional and mental disorders in days makes 50-200 mg.

At psychosomatic frustration in day it is necessary to accept 10-75 mg.

If necessary to suppress a severe itch the size of a dose is established in an individual order, however, it should not be more than 200 mg.

Children at the age of 4-7 flyings a day need to accept 10-20 mg (in two-three receptions); 8-14 years – 20-30 mg (in three steps), 15-18 years – 30-50 mg (in three steps).

Overdose

Overdose by drug has the following symptoms:

  • change of an ECG, fibrillation of ventricles, arrhythmias, ventricular tachycardia, lengthening of intervals of PR and QT, bradycardia, sinus tachycardia, pressure decrease oppression of function of a myocardium;
  • sedation, hypothermia, coma, confusion of consciousness, extrapyramidal frustration, hyperthermia, agitation, areflexia;
  • ischuria, mydriasis, xeroderma, nose congestion, miosis, sight illegibility;
  • apnoea, respiratory depression, fluid lungs;
  • lock, lowering of motility of a gastrointestinal tract, intestinal impassability;
  • uraemia, oliguria.

Manifestation of toxicity comes at concentration of thioridazine more than 10 mg/l in plasma, the death occurs at concentration of 20-80 mg/l.

For treatment it is necessary to adjust lung ventilation and adequate oxygenation. It is necessary to carry out an ECG. A therapy essence in correction of electrolytic disturbances, it is necessary to enter lidocaine, being careful at the same time as there is a risk of emergence of spasms, Isoproterenolum, Phenytoinum. It it is necessary to apply quinidine, procaineamide and Disopyramidum as they can influence increase in an interval of QT.

For suppression of acute ektrapiramidalny frustration it is necessary to use trigeksifenidit or difengidramin.

Interaction

Sonapaks's reception can strengthen action hypotensive, analgeziruyushchy, narcotic and hypnagogues, and also alcohol; to increase anticholinergic effect of amitriptyline.

Sonapaks reduces effect of amphetamine, a levodopa.

The drug combination with ephedrine can cause hypotension. Anti-steroid means in a combination with this drug increase risk of emergence of an agranulocytosis.

Lengthening of an interval of QT can be caused by reception of probucol, a tsizaprid, astemizol, Pimozidum, erythromycin, quinidine and Disopyramidum that can lead to development of ventricular tachycardia.
