Structure

In 1 tablet of an atorvastatin of calcium 10, 20, 40 of mg.

Croscarmellose sodium, hypro rod, polysorbate, lactoses monohydrate, magnesium stearate, magnesium oxide, silicon dioxide, cellulose as excipients.

Release form

Tablets in a film cover of 10, 20 and 40 mg in packaging on 30, 60 or 90 pieces.

Pharmacological action

Decrease in level of lipids of blood.

Pharmacodynamics

Atorvastatin — the selection inhibitor of the GMG-KOA-reduktazy which is taking part in synthesis of cholesterol in a liver. Besides, drug promotes increase in number of receptors — LPNP on cells that attracts strengthening of capture and a metabolism of LPNP. Promotes decrease in level of cholesterol, lipoproteins, holesterina-LPNP, apolipoprotein - Century.

At patients with a hypercholesterolemia (family, primary) and the mixed lipidemia reduces level holesterina-LPONP and TG.

In a dose of 40 mg drug reduces the LPNP level by 50%, cholesterol for 37%, triglycerides for 29%, and apo-V for 42%. Depending on a dose reduces the LPNP level at the family hypercholesterolemia steady against treatment by other hypolipidemic means. The effect appears on the third week from an initiation of treatment, and the maximum of effect is reached in a month.

Pharmacokinetics

Absorption level high. Cmax in blood is defined by 2 h. Decrease in level holesterina-LPNP has no dependence on time of administration of drug (morning or evening). There is a dependence between a dose and extent of absorption.

Bioavailability of low-12% that is connected with a presistemny metabolism in a gastrointestinal tract. Communication with blood proteins of 98%. It is metabolized in a liver under the influence of isoenzymes with formation of active metabolites. Activity of metabolites remains 20-30 hours. It is removed through intestines. T1/2 — 14 hours. In urine 2% of the accepted dose are defined.

Indications to use

  • the combined lipidemia (IIb type);
  • family hypercholesterolemia (as addition to a diet);
  • primary hypercholesterolemia (IIA type).

Contraindications

  • liver diseases;
  • hypersensitivity;
  • myopathy;
  • pregnancy and feeding by a breast;
  • age till 18 flyings;
  • lactose intolerance.

With care it is appointed at a hyperthyroidism, arterial hypotension, sepsis, disturbances of electrolytic balance.

Tulip, application instruction (Way and dosage)

Drug is used inside at any time.

The dose of drug depends on cholesterol level at the patient and fluctuates from 10 mg to 80 mg a day. 10 mg or 20 mg are most often appointed. Maximum SD of 80 mg.

Each 3-4 weeks the level of lipids is controlled and the drug dose is adjusted. At patients with an impaired renal function the dose is not adjusted. At insufficiency of function of a liver the dose decreases at a constant control of transaminases (ACT and ALT).

Overdose

In case of overdose strengthening of side reactions takes place. There is no specific antidote.
The symptomatic treatment is carried out.

Interaction

At use with fibrata, erythromycin, cyclosporine, klaritromitsiny, antifungal and immunodepressive drugs, niacin the risk of a myopathy increases.

Inhibitors of an isoenzyme CYP3A4 cause increase in concentration of an atorvastatin.

Cyclosporine increases bioavailability and concentration of an atorvastatin. Erythromycin, diltiazem and klaritromitsin also increase concentration of drug. Itrakonazol will see off to increase in AUC of an atorvastatin by 3 times.

The excessive use of grapefruit juice causes increase in concentration of an atorvastatin.

Combined use with efavirenzy or rifampicin attracts decrease in concentration of drug.

Antacids reduce concentration of this drug by 35%.
The hypolipidemic effect at reception with kolestipoly is much higher, than each drug separately.

At use of digoxin and an atorvastatin in high doses concentration of digoxin increases. Atorvastatin shortens a prothrombin time at combined use with warfarinAmlodipin does not influence pharmacokinetics of this drug.

The myopathy meets seldom and is shown by pain and weakness in muscles. All these temporary phenomena also disappear after a drug dose decline.
